Required Forms for Kindergartners

These 2023-24 Forms are Required Prior to the First Day of School

1) Birth Certificate

If you did not already submit your child’s birth certificate with your enrollment form, please:

* Upload a copy via this form

2) Immunization Records 

  •  
    • PDF Form: Print and complete the .pdf below
      • OR
    • Form from Clinic: Your doctor/clinic can fax or email the required information.

Make sure to sign and date side 2 of this form as appropriate.

For students transferring from another school, immunization records should be forwarded from your former school.

3) Early Childhood Screening Form

    • Complete the screening with your resident school district, and send the form to River Grove. The form is available from your resident district.

What is Early Childhood Screening?

Any child who will go to school in the state of Minnesota must have an Early Childhood Screening record before entering kindergarten.

Children usually go through screening one to two years before they enter kindergarten. However, if your child is five years old, not eligible for kindergarten, and not yet been through preschool screening, call to arrange an appointment.

Parents must schedule Early Childhood Screening and registration appointments within your own resident school district.

If you are unsure as to whether your child is on the census, you may check with the Stillwater’s Enrollment Office at 651-351-8412.

Appointments for the screening typically last one hour and (for District 834 residents) are held at the Early Childhood Family Center, 1111 Holcombe Street S., Stillwater MN 55082, and can be made by calling 651-351-4022. Please schedule your appointment prior to the end of this school year.

If you do not live within Stillwater District 834, check with your own resident school district’s enrollment office.

To send River Grove a copy of your child’s Early Childhood Screening report please:

4) General Health Form

  •  
    • PDF Form: A paper copy must be printed and completed. Physician or healthcare provider signature is required.

This form must have a physician’s or health care provider’s signature.

This form is required for all kindergartners and new students. For grades 1-6, it is required only if your child has special medical needs or receives medication at school.

5) Physical Activities and Assumption of Risk

    • On-line Form: Complete and submit online.

Your student will be participating in physical activities associated with River Grove. Physical activities require each participant’s parent or guardian to sign this acknowledgment and assumption of risk.

6) Topical OTC Medication Form

  •  
    • On-line Form: Complete and submit online.

This form is for sunscreen, insect repellent, lip balm, and over-the-counter (OTC) topical medication permission.

NOTE: This form is not for pain relievers such as Tylenol and Advil. (See Optional Forms page for pain relievers permission.)

How to Turn in Completed Forms

There are two types of forms:

  • 1) Forms in .pdf format generally require signatures from physicians or other personnel. For the .pdf forms, please submit to River Grove in one of these ways:
  • 2) On-line forms are sent automatically to River Grove. You do not need to print and send them into the office.
    • Please complete the on-line forms on a computer, not a smart phone.
